Job Responsibilities
Minimum Qualifications
Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification.
Preferred: BSN, Previous experience in home care and/or medical case management, Newborn and/or Pediatric. Bilingual-Spanish.

Job Responsibilities
- Performs job responsibilities with minimal supervision
- Provides home visitation to at-risk infants, children, teens, and young adults
- Serves as job expert or organization resource in the C1st/CMS programs
- Consults and coordinates with health care team members to assess, plan,
- Maintains accurate, detailed reports and client records
- Modifies client treatment plans as indicated by clients' responses and conditions
- Assess clients’ health care needs, may provide referrals to other programs for
